On the morning of March 29, information from Nghe An Obstetrics and Children’s Hospital said that the hospital had just received emergency care for an 18-month-old girl with a rare simple ovarian torsion˳

According to the patient’s family, 3 days after being admitted to the hospital, the HTU patient (18 months, Quynh Luu) had a high fever of over 38˳5 degrees, crying˳ The family gave her fever-reducing medicine at home, but it didn’t help, so she took her to the Nghe An Obstetrics and Children’s Hospital˳

Through examination, the doctor ordered to do paraclinical tests to support the diagnosis˳ At the functional exploration department, the doctor conducted an abdominal ultrasound for the patient˳ Through ultrasound, the size of the baby’s right ovary is 3cm˳ This is an abnormal size, not suitable for the child’s age˳

The right ovary no longer shows the signal of feeding blood vessels, the ovarian parenchyma is abnormally thick, the follicles are pushed to the periphery, and there is an infiltrate around˳

The ultrasound image suggested a simple torsion ovary, with no associated ovarian tumours˳

The patient was transferred to emergency surgery, the general surgery team performed laparoscopic detorsion˳ Unfortunately, after about 30 minutes of removal, the right ovary does not turn pink again˳ Determined due to late detection, the patient’s ovary was necrotic, unable to recover, the doctor was forced to remove the right ovary˳

Doctors say that ovarian torsion can occur at any age, but is rare in young children, and that ovarian torsion is rare compared to ovarian cystic torsion˳

Currently, there are no complete statistics on the incidence in children under 1-2 years old, but in fact, in clinical examination in Vietnam, there are very few reports of ovarian torsion at this age˳ One year ago, Nghe An Obstetrics and Pediatrics Hospital recorded a case of ovarian torsion in a 3-month-old baby˳

Illness does not exclude anyone

Doctor Duong Ngoc Van, Medlatec Hospital, said ovarian torsion will block blood circulation to the ovary and fallopian tubes, causing tissue death around the ovary, leading to ovarian necrosis˳ Many cases of severe gangrene require surgical removal of the ovaries˳

Ovarian torsion in women does not exclude any age, from infancy to premenopausal women can be affected˳ However, it is most common between the ages of 20 and 40˳

There are also other reasons such as:

• Ovarian cyst, dermoid cyst (teratoma), simple cyst, hemorrhagic cyst, the larger the tumor size, the higher the risk˳

• Patients with a history of surgery in the pelvic and pelvic areas are also at risk of ovarian torsion˳

• Women undergoing ovarian stimulation have a higher risk of ovarian torsion˳

• When pregnant women have ovarian cysts, hormone levels are higher than normal, causing the surrounding ligaments to stretch and twist into the ovary˳ Especially in the first 3 months of pregnancy, this is also the time when the ovaries are prone to torsion˳

Worryingly, the clinical symptoms of ovarian torsion are unclear and varied, depending on the time, location, extent and frequency of torsion˳ Symptoms of ovarian torsion may include:

Severe and sudden pelvic pain: pain is often constant or sometimes intermittent˳ Pain often does not improve with conventional pain relievers˳ In cases where the ovaries untwist on their own, the pain may be relieved˳

– Vomiting and nausea˳

– Fever: usually in the late stage˳

“However, these signs are similar to diseases: kidney stones, appendicitis, urinary tract infections, gastroenteritis, so it is difficult to diagnose,” Dr˳ Van emphasized˳

In addition, there are other symptoms such as indigestion, constipation, lower extremity edema, making daily activities and life difficult for patients˳ Therefore, it is necessary to see a doctor for a check-up every time you notice any unusual symptoms like these˳
