In the past three years, she has been diagnosed with malignant bladder tumors and frequently receives treatment at Bình Dân Hospital in Ho Chi Minh City.
Mr. Long wonders: “It’s very difficult for seriously ill patients like my grandmother to request a referral for treatment. Can I change her initial medical examination and treatment facility to Bình Dân Hospital for convenience?”

According to the Vietnam Social Security, the registration process for medical examination and treatment under health insurance is governed by Clause 2, Article 26 of the Health Insurance Law. Accordingly, individuals participating in health insurance can change their initial registered medical examination and treatment facility at the beginning of each quarter.
On August 15, 2023, the Vietnam Social Security issued Consolidated Document No. 2525/VBHN-BHXH, which provides detailed guidance in Clause 3, Article 47.137.
Thus, Mr. Long can only proceed with changing his grandmother’s initial health insurance medical examination and treatment registration at the start of each quarter. The beginning of each quarter corresponds to January, April, July, and October.
The required documents for changing the initial medical examination and treatment registration include two types. First, the Declaration Form for Providing and Changing Information of Health Insurance Participants (Form TK1-TS, 1 copy/person); or the List of Requests to Change Information of Health Insurance Participants (Form D07-TS, 1 copy) for submissions made by organizations. Second, the old valid health insurance card.
According to the Vietnam Social Security, the processing time for these documents is within 3 working days.
Employees under labor contracts should submit their documents to the Social Security Office or through their workplace. Other cases must submit their documents to the agency that issued the health insurance card.
The Vietnam Social Security stated: “Currently, individuals participating in health insurance who need to replace their health insurance card due to changes in their initial medical examination and treatment facility are not required to pay any fees.”
